本同意書說明本活動將如何處理本表單所蒐集到的個人資料。當您勾選「我同意」並簽署本同意書時，表示您已閱讀、瞭解並同意接受本同意書之所有內容及其後修改變更規定。

一、基本資料之蒐集、更新及保管

1. 本活動蒐集您的個人資料在中華民國「個人資料保護法」與相關法令之規範下，蒐集、處理及利用您的個人資料。
2. 請於申請時提供您本人正確、最新及完整的個人資料。
3. 本活動因執行業務所蒐集您的個人資料包括姓名、電話、 email、身分證字號等。
4. 若您的個人資料有任何異動，請主動向本所申請更正，使其保持正確、最新及完整。
5. 若您提供錯誤、不 實、過時或不完整或具誤導性的資料，您將損失相關權益。
6. 您可依中華民國「個人資料保護法」，就您的個人資料行使以下權利：

   (1) 請求查詢或閱覽。 (2)製給複製本。 (3)請求補充或更正。
   (4)請求停止蒐集、處理及利用。 (5)請求刪除。但因本活動執行職務或業務所必須者，本所得拒絕之。若您欲執行上述權利時，請與本所連繫。但因您行使上述權利，而導致活動權益受損時，本所將不負相關賠償責任。

二、蒐集個人資料之目的

1. 本所為執行活動相關業務需蒐集您的個人資料。
2. 您可以拒絕向本活動提供個人資料，但您可能因 此喪失您的權益。
3. 本所利用您的個人資料期間為即日起 1年內，利用地區為台灣地區，於次年定期銷毀所填具之報名表。

三、基本資料之保密

您的個人資料受到本所之保護及規範。本所如違反「個人資料保護法」規定或因天災、事變或其他不可抗力所致者，致您的個人資料被竊取、洩漏、竄改、遭其他侵害者，本所將於查明後以電話、信函、電子郵件或網站公告等方法，擇適當方式通知您。

四、同意書之效力

1. 當您勾選「我同意」並簽署本同意書時，即表示您已閱讀、瞭解並同意本同意書之所有內容，您如違反下列條款時，本所得隨時終止對您所提 供之所有權益。
2. 本所保留隨時修改本同意書規範之權利，本所將於修改規範時，於本活動網頁 (站 )公告修改之事實，不另作個別通知。
3. 您自本同意書取得的任何建議或資訊，無論是書面或口頭形式，除非本同意書條款有明確規定，均不構成本同意條款以外之任何保證。

五、準據法與管轄法院

本同意書之解釋與適用，以及本同意書有關之爭議，均應依照中華民國法律予以處理，並以臺灣臺北地方法院為管轄法院。

This Consent Form explains how the Event will handle the personal information collected on this form. By checking the “I agree” box and signing this Consent Form, you acknowledge that you have read, understood, and agree to accept all of the contents of this Consent Form and any subsequent changes and amendments.

1. Collection, Update, and Storage of Basic Information

1. We collect, process, and use your personal information for the Event according to the provisions of the Personal Data Protection Act and related laws and regulations of the Republic of China.
2. When filling out an application, please provide accurate, up-to-date, and complete personal information.
3. Your personal information, including name, telephone number, email address, and ID card number, shall be collected for the purpose of conducting the Event.
4. If there is any change in your personal information, please actively request a correction from the University to ensure its accuracy, up-to-dateness, and completeness.
5. If you provide incorrect, inaccurate, outdated, incomplete, or misleading information, you will lose your rights.
6. You may exercise the following rights with respect to your personal information in accordance with the Personal Data Protection Act of the Republic of China:

   (1) To request access to or review of your personal information. (2) To make a copy of your personal information. (3) Request additions or corrections. (4) Request to stop the collection, processing, and use. (5) Request for deletion. However, the University may deny requests if such information is necessary for the performance of the Event or its operational activities. If you wish to exercise any of the above rights, please contact the University. However, The Institute will not be liable for any damages caused by the exercise of your rights as described above.

2. Purpose for the Collection of Personal Information

1. The Institute needs to collect personal information from you in order to carry out activities related to the Event.
2. You may refuse to provide your personal information to the Event, but you may lose your rights and interests as a result.
3. The Institute will use your personal information for a period of one year from now. The region of use shall be Taiwan. The completed application form will be destroyed as scheduled in the following year.

3. Basic Information Confidentiality

Your personal information is subject to the protection and regulations of The Institute. If the University violates the Personal Data Protection Act or causes your personal information to be stolen, leaked, altered, or otherwise violated due to natural disasters, unforeseen events, or other force majeure events, The Institute will notify you via telephone, letter, email, or website announcement as appropriate.

Legal Effect of the Consent Form

1. By checking the “I agree” box and signing this Consent Form, you acknowledge that you have read, understood, and agree to accept all of the contents of this Consent Form. The Institute may terminate all rights and interests provided to you at any time if you breach any of the following terms and conditions.
2. The Institute reserves the right to modify the terms and conditions of this Consent Form at any time, and will post a notice of such modifications on the Event website without any additional notice.
3. No advice or information, whether in written or oral format, obtained by you from this Consent Form shall constitute any guarantee outside the terms of this Consent Form, except as expressly provided in the terms of this Consent Form.

Governing Law and Competent Court

The interpretation or application of this Consent Form and any disputes derived from it shall be governed by the laws of the Republic of China, and the Taiwan Taipei District Court shall be the competent court.

本同意書は、本フォームにより収集した個人情報の本イベントにおける取扱方法について説明するものです。「同意します」にチェックを入れ、本同意書に署名することにより、利用者が本同意書の全ての内容をすでに読み、それらを理解し、それらの内容及び以後修正及び変更されることを受け入れることに同意したものと見なします。

一、基本情報の収集、更新、保管

1. 本イベントにおける個人情報の収集は、中華民国の「個人情報保護法」及び関連法令の規範のもとで、利用者の個人情報を収集、処理、利用するものです。
2. 申請時には、利用者本人の正確且つ完全で、最新の個人情報を提供してください。
3. 本イベントにおける業務執行のために収集する個人情報には、氏名、電話番号、メールアドレス、身分証明書番号などが含まれます。
4. 個人情報になんらかの変更が生じた場合、当所へと変更を自主的に申請し、情報の正確性、最新性、完全性を維持してください。
5. 誤りや虚偽の含まれる情報、または現在のものとは異なる情報を提供した場合や、不完全な情報、または誤解を招く可能性のある情報を提供した場合、利用者はその関連する権利及び利益を喪失します。
6. 利用者は、中華民国の「個人情報保護法」に基づき、個人情報について以下の権利を行使することができます。

   （1）照会または閲覧の請求。（2）複製本の作成。（3）補足または訂正の請求。 （4）収集、取り扱い、利用の停止の請求。（5）削除の請求。ただし、本イベントにおける職務または業務の執行に必要とされる場合、当所はこれを拒否することができるものとします。上記の権利の行使を望む場合は、当所までご連絡ください。ただし、上記の権利を行使したために、イベントに関する権利及び利益に損害が及んだ場合、当所は賠償責任を負いません。

二、個人情報収集の目的

1. 当所は、イベントに関連する業務を執行するために利用者の個人情報を収集します。
2. 利用者は個人情報の提供を拒否することができますが、それにより権利及び利益を喪失する可能性があります。
3. 当所が利用者の個人情報を利用する期間は即日から1年以内とし、個人情報を利用する地区は台湾地区です。また、記入された申込書は翌年の所定の時期に廃棄されます。

三、基本情報の秘密保持

当所は、利用者の個人情報を保護し、規範に基づいて取り扱いを行います。当所が「個人情報保護法」の規定に違反した場合、または自然災害、事変またはその他の不可抗力により利用者の個人情報が窃取、漏洩、改ざん、その他の侵害を被った場合、当所は事実を調査した後で、電話、書状、電子メールまたはウェブサイトの公告等の適切な方法により、利用者へと通知を行います。

四、同意書の効力

1. 「同意します」にチェックを入れ、本同意書に署名することにより、利用者が本同意書の全ての内容をすでに読み、それらを理解し、それらの内容に同意したものと見なし、利用者が以下の条項に違反した場合、当所は、利用者に対して提供した全ての権利及び利益を随時終了することができます。
2. 当所は、本同意書の規範における権利を随時修正する権利を有します。規範を修正する場合、本イベントのウェブページ（ウェブサイト）で修正の事実について公告するものとし、別途個別での通知を行うことはありません。
3. 利用者が本同意書により取得したいかなる提案または情報についても、書面または口頭での形式を問わず、本同意書の条項により明確に規定されている場合を除き、本同意書の条項以外のいかなる保証も構成することはありません。

五、準拠法及び管轄裁判所

本同意書の解釈及び適用、並びに本同意書に関連する紛争については、全て中華民国の法律に従って処理するとともに、台湾台北地方裁判所を管轄裁判所とします。
